Major authors...

Ermberto's Incestuously Anal: just about everything on the site. Although it's very silly, formulaic stroke material, it usually gets me off anyway.
For that matter, Frank McCoy is usually reliable stroke, although too pedo for my tastes; I have to mentally add years to the characters when reading.
Phil Phantom was a prodigious and skilled author for whom story codes were impossible - everything and anything is in there. Although I have to mentally skip over the stuff that I don't like, I've probably orgasmed more times to Phil than any other author. His writings are now scattered; the mammoth See No Evil is on asstr, his Guide to Writing Good Trash is a humorous classic that motivated me to try writing.
I'm a big fan of PlanetDweller. Although I skip around his frequent inclusion of menstrual sex (ick), it doesn't detract from his well-constructed and fantastically perverse writing. His novel-sized series The Body Worker is a masterpiece of the genre!
Art Martin is a good writer whose tastes are very close to my own. All his stories are worthwhile, although a few have a bit more mm than I care for. My favorite is Prof. Litcher's Pillow Maid (M+f, inc, con, oral, anal, creampie)
I recently found the wet amber archive, and excluding some of them as not my tastes (underage, scat, nc..) there are some decent Ff-inc in some stories there.

A handful of individual stories...

Ale Stone: Pears And Strawberries (FmfF inc, preg) has a long slow buildup and then wham, rapid acceleration close to the end.
Meme Mispelt: Blame it on "Ed" (F FF MF MMF MFF cons toys oral anal) is funny and fun to read, despite lack of inc.
Fate by emeritus (MF, FF, Mf, mF, MM, cons, incest, bi, cheat, swing, group, orgy, exhib, oral, anal, mast)
Lo and Behold (family inc) by Diola Dragontail. An Asian family with surprisingly well-written characters and pacing. There is one pre-pubescent girl and some Fm-inc that's not to my tastes, but otherwise a nice series.
Over on SOL, I found the author Vulgus who, despite a lot of nc that's too rough for me, also wrote some very nice stories with Ff-inc, such as The Bitch Next Door, which is nicer than the title sounds.
Very old stuff
- For some reason, Ana's Cabin (MF, MFf, Mf, Fm, MFfm, inc, ws, cons) by silentkiller has a special place in my memory. The Ff-inc is brief but nice.
- business-first, although it's poorly-written stroke material. I probably liked it because of all the Ffff inc.
- Wanton Family is a good example of a classic, timeless, grade A, dumb stroke story. Hey, it works.

Reviews of other longer stories of note...

Dorsai has quite a number of stories that push my buttons well (group/anal/Ff-inc) including the massive Next Door Neighbor. The problem is that the good bits are buried in a long, bothersome backstory about business, veterans, the mob, blah blah blah. The Ff-inc doesn't happen until 94,000 words into a 200,000 word novel.
Nick Scipio's Summer Camp. Another massive novel-length story, and this time the characters and surrounding story are actually well-written, which sets up the reader well for the hot sex when it finally happens. The most risqué scenes are in Book 2, around Chapter 37 - mf inc oral, ff inc, and some almost Ff inc.
Pryn and Proper Daughters (mdom, inc, mf, Mf, mF, Mff and mff) is a fascinating parallel to my own story, Mira - a twisted family, a father involved with criminals who dominates his family sexually, etc. The main differences are 1. 'Dutchboy' is a better writer than me, 2. He write more Mf and mF incest, I write more Ff and ff, and 3. My story is lighter in tone, less misogynist language, happier, and has a happy ending.
Wolf Summers by ElSol is not just a sexual fantasy, but a young man's power fantasy of domination in every way - emotional, physical, financial, personal. I enjoyed it a lot although there are long stretches between the sex scenes, and he skirts around FF-inc many times without ever really getting into it.

International

If you like the international theme of my stories Mira, Singapore, Three Countries, then you might enjoy:

- Baykit by Leslie Schmidt (five parts, MF, Mf, mg, almost Ff inc in one brief point); caution: pedo
- VeryWellAged's "Jake" stories, which mostly feature an American man (60-ish) having sex with Filipina girls/women (12 and up, frequently virgins); there is some Ff inc in some of the story arcs (Joyfully, The Package Deal, The Rainy Season) which is nice but, as usual, too brief for my tastes.
